Everything for your heart - Screening program to identify patients with cardiovascular risk factors.

Duration


Over a period of 4 years (July 2020 - December 2023), the project partners will carry out activities to promote and support a healthy lifestyle, as well as to prevent and treat heart disease among the population of the 8 regions Development.

The objective of the project


The objective of the project is: promoting social inclusion, combating poverty and any form of discrimination, by increasing access to accessible, sustainable and high quality services, including health care and social services of general interest, by organizing health programs and targeted services screening to identify patients at cardiovascular risk, for 165,001 people, of which at least 50% belong to vulnerable groups, as well as organizing training programs for 900 people to ensure an improved level of skills of professionals involved in screening for patients with cardiovascular risk.

The specific objectives of the project are:


SO 1. Ensuring a unitary standard for screening at national level in order to identify persons at cardiovascular risk, by developing a unique methodology, applicable at national level, for screening the population by medical methods, with the main role of detecting the disease still from the early stages of installation.

SO.2 Improving the level of theoretical and practical knowledge through activities to support the increase of the technical capacity of the staff involved in screening for the identification of patients at cardiovascular risk, through the provision of specific training programs for 900 people (staff involved in providing services offered through the national screening program for the identification of patients with cardiovascular risk factors) in the form of accredited, theoretical and practical medical education courses, ensuring a high level of training by developing an accredited training curriculum based on the methodology at national level and the consolidation and integrated approach, of high quality level, of the working methods and the increase of the degree of involvement of the Romanian specialists in the European initiatives for information exchange in the field of cardiovascular screening.

SO.3. Improving the health of the population by conducting a complex screening program to identify people at cardiovascular risk for 165,001 people identified in the project, especially from vulnerable groups and optimizing diagnosis and treatment for 4,000 people identified directly or by passive referral, by providing services and means of support to increase access to specialized medical services and treatment, including peer support, psychological counseling, social assistance, by periodically re-evaluating patients and developing reports, studies and public policy proposals based on data obtained in following the screening.

SO 4. Awareness and promotion of the need for the screening program to identify people at cardiovascular risk by conducting specific information, education and awareness activities for target groups and the general public, at the level of individual, group and community, taking into account that access to services depends on the addressability of the population and the degree of awareness of their own health needs and rights to health services.

SO.5 Promoting social inclusion, combating poverty and any form of discrimination, with the priority of increasing access to sustainable and high quality services, through the use of methods and tools of social innovation in training sessions, dissemination to at least 900 professionals in the medical sector of the notions and principles related to non-discrimination and equal opportunities as part of training programs, embodied in separate chapters and examples of good practice, as well as by conducting a screening program at national level, non-discriminatory and adapted to the target group for 165,001 people, respectively facilitating the access to specialized services and treatment for 4000 people, within which methods and tools of social innovation will be used and the principles of non-discrimination, equal opportunities and the use of ICT in the interventions will be promoted.

Among the expected results of the project we mention: 165,001 persons benefiting from screening for cardiovascular risk; opening of 8 regional cardiovascular screening centers; 165,001 people mobilized to the family doctor; 4000 people who benefit from accompanying measures for cardiovascular screening; 4000 people with referral to specialist; elaboration of the cardiovascular screening methodology for the identification of patients who present high risk factors; elaboration of a protocol for the support services provided to the target group; elaboration of situation reports extracted from the National Register of Cardiovascular Risk Patients; elaboration of a nationally accredited training curriculum; information and awareness campaign carried out at national, regional / local level.

The total value of the project is 96,544,958.23 lei, of which EU funding is 81,335,377.53 lei, and the non-reimbursable value is 88,528,451.63 lei.

The project is co-financed from the European Social Fund through the Human Capital Operational Program 2014-2020.

ACTIVITIES


ACTIVITY A1 - ELABORATION OF THE METHODOLOGY FOR THE NATIONAL SCREENING PROGRAM FOR THE IDENTIFICATION OF PATIENTS WITH CARDIOVASCULAR RISK FACTORS

A1.1. Development of the methodological framework necessary for the implementation of the national screening program for the identification of patients with cardiovascular risk factors (eg the use of the Score calculation model of the European Society of Cardiology, in accordance with the Recommendations of the European Cardiovascular Disease Prevention Guide)

Description: This activity involves organizing a framework for developing strategic documents. Working groups will be organized, meetings for the development of materials and for obtaining consensus for:

  1. substantiated screening methodology,
  2. population screening strategy in order to optimally apply cardiovascular prevention measures,
  3. individual assessment sheet of persons,
  4. the protocol for support services,
  5. statistical information form adapted to the circuit / course of the tested person,
  6. format (template) of annual report

A1.2. Computerization activities of the screening databases, integrated with the computer systems of the national program (eg national registers), with online password access of all providers in order to identify the CNP of the persons tested in the screening program to identify patients with cardiovascular risk factors.

Description: This activity supposes the establishment of the criteria and technical specifications necessary for the accomplishment of the Register of evidence of the persons identified with cardiovascular risk, the organization of working groups for the elaboration of the architecture of this register. Also, it will intervene with feedback to the development of the application, testing, elaboration of subsequent modifications for the development of the application.

A1.3. National monitoring and control of the implementation of the screening program for the identification of patients with cardiovascular risk factors.

Description: The activity involves the elaboration of the methodology of monitoring and internal evaluation of the project to ensure the analysis and control of the achievement of the objectives and results within the assumed time limits. For the elaboration of this methodology, the following actions will be performed:

  • Organizing working groups with experts involved for the elaboration of the monitoring methodology at national level and of the necessary tools and for the implementation control;
  • Realization and dissemination of the monitoring methodology and of the necessary tools at national level and for the control of the implementation;
  • Carrying out the activity of monitoring the implementation of the screening program;
  • Elaboration of quarterly / periodic monitoring reports.

Description: The activity involves the preparation of a quarterly report which will highlight the results of national screening in each of the 8 development regions concerned.
For the implementation of the activity, the following actions will be undertaken:

  • Organizing the working group / groups for the elaboration of the structure related to the analyzes at national level;
  • Elaboration of the structure related to the analyzes at national level;
  • Carrying out the activity of elaborating analyzes at national level of the obtained data;
  • Elaboration of the national guide of good practices for the implementation of the national cardiovascular screening program.

ACTIVITY A2 - PROVIDING SPECIFIC TRAINING / TRAINING PROGRAMS FOR 900 SPECIALISTS INVOLVED IN THE NATIONAL SCREENING PROGRAM FOR IDENTIFYING PATIENTS WITH CARDIOVASCULAR RISK FACTORS

A2.1 Elaboration of a training curriculum based on the developed methodology and its accreditation according to the norms in force

Description:Within this activity, a training program will be carried out which will be accredited at national level. The following actions will be taken:

  • Curriculum development for classic training sessions;
  • Adapting the curriculum for classic training sessions for e-learning and webcasting;
  • Accreditation and crediting of training courses.

A2.2Carrying out the training / training program / programs for 900 family doctors involved in carrying out the national screening program for identifying patients with cardiovascular risk factors.

Description:Within this activity, a specific professional training program will be carried out, with elements that facilitate teamwork, with doctor-patient communication components, including both theory and practice hours, especially since the field requires an integrated approach. The training course will take place under the conditions of national accreditation and individual accreditation of each session.
The course topics will be constantly updated to correspond to the latest discoveries in the field.

ACTIVITY 3 - PROVISION OF MEDICAL SERVICES FOR 165,001 PERSONS THROUGH THE NATIONAL SCREENING PROGRAM FOR THE IDENTIFICATION OF PATIENTS WITH CARDIOVASCULAR RISK FACTORS

A3.1 Acquisition of the equipment necessary to carry out the screening activities / sub-activities of 165,001 persons for the identification of patients with cardiovascular risk factors.

Description:The following actions will take place within this activity:

  • Substantiation of the real needs of screening equipment
  • Elaboration of technical specifications and documentation related to the acquisition of equipment
  • Carrying out the acquisition of 900 packages of screening equipment for family doctors, which contain thallium scale, sphygmomanometer.

A3.2 Identification / mobilization and support activities for 165,001 people target group, consisting of people who will benefit from the national screening program to identify patients with cardiovascular risk factors, as well as accompanying 4,000 people with referral to a specialist

Description: This activity will have two components:

  • AActivities to identify / mobilize the WG and to facilitate (support) at least 165,001 people at the national CV screening program;
  • Support, mobilization and support activities in order to facilitate access for people with high CV risk to specialized medical services in screening centers / other specialized services.

A3.3 Carrying out the screening for a number of 165,001 persons from the target group in order to identify the persons at cardiovascular risk.

Description:The following actions will be carried out in this activity:

  • identification of persons with cardiovascular risk factors,
  • entering the identification data, as well as the data resulting from the evaluation of the eligible persons in the common database, available online,
  • anamnesis, collection of heredocolateral and pathological antecedents (considered significant),
  • clinical examination of eligible persons,
  • identification and investigation of risk and accumulation of risk factors,
  • application of lifestyle change measures, individualized, depending on the size of the established risk,
  • centralization and monthly reporting of recorded data.

A3.4 Carrying out 165,001 investigations necessary to identify the cardiovascular risk

Description: This activity involves the following actions:

  • Acquisition of medical analysis services for 165,001 persons, including transport, eGFR and transmission of results.
  • Performing medical appointments and tests: complete lipid profile, fasting blood glucose, serum creatinine, glycated hemoglobin.
  • Receiving and centralizing the results of the analyzes.

A3.5 Assessment of psychosocial risk factors (based on questionnaire) in patients with high cardiovascular risk (approximately 5% of the total number of screened persons), in order to identify possible barriers to lifestyle change and adherence to treatment.

Description: Within this activity, the following actions will be undertaken:

  • Development of the questionnaire and its pre-testing, in order to establish its addressability
  • Application of the questionnaire
  • Entering the data from the questionnaires and analyzing them
  • Elaboration of a quarterly evaluation report of psycho-social risk factors

A3.6 Support activities for conducting national level screening programs in order to identify patients with cardiovascular risk factors (eg insurance costs related to travel in communities for staff involved in screening, etc.)

Description: Within this activity, the following actions will be undertaken:

  • Development of the methodology for providing patient support services, with a focus on patients with high CV risk (including referral to specialized services)
  • Carrying out visits in the territory for the staff involved in carrying out the screening

ACTIVITY A4: - INFORMATION, EDUCATION, AWARENESS ACTIVITIES OF 165,001 PEOPLE WHO ARE PART OF THE TARGET GROUP OF SCREENING SERVICES FOR THE IDENTIFICATION OF PATIENTS WITH CARDIOVASCULAR RISK FACTORS

A4.1 Information, education, awareness activities of 165,001 people who are part of the target group of screening services for the identification of patients with cardiovascular risk factors

Descriere: This activity involves the development of a communication plan that aims to ensure general access to information, data, processes generated by implementing the Cardiovascular Disease Screening project for the target audience, to provide the necessary support for the implementation of the Cardiovascular Disease Screening project objectives and to increase awareness for the group target, , but also of the general public regarding the activities, objectives and achievements of the Cardiovascular Disease Screening project.
The communication strategy and mix will be established following a market study. The communication process will have a section dedicated to family doctors. Communication dedicated to the population at risk will use different communication channels.

ACTIVITY 5 - SUPPORT FOR THE PROVISION OF SCREENING SERVICES FOR THE IDENTIFICATION OF PATIENTS WITH CARDIOVASCULAR RISK FACTORS BY SUPPORTING THE ESTABLISHMENT AND OPERATION OF 8 SCREENING CENTERS AT REGIONAL LEVEL

A5.1 Support for the provision of screening services for the identification of patients with cardiovascular risk factors by supporting the establishment and operation of 8 screening centers at regional level..

Description:Within this activity, 8 regional screening centers will be set up in some hospitals that will be identified within the screening methodology. The regional screening centers will serve all the counties of the country and will complete the need for specialized cardiology services, as well as performing population screening in the cardiovascular field. The activity involves the development of a screening guide and the necessary modern equipment.

ACTIVITY 6 - GENERAL PROJECT MANAGEMENT

A6.1 General project management - organization, establishment, development of procedures and activities for project implementation according to the funding application, with the participation of the applicant, organization of the project team, management meetings and ensuring the connection with the managing authority.

Description: This activity refers to the monitoring and evaluation of the evolution of the project implementation - technical and financial reporting, preparation of reimbursement requests, payment requests, evaluation of activity reports, accounting expertise, audit, beneficiary feedback, document archiving.
